McCleod syndrome vs Congeital Lobar Emphysema

Swyer-James/McCleod Syndrome
Congenital Lobar Emphysema
1
Recurrent childhood acute bronchiolitis => Obliterative bronchiolitis (adeno-virus)
Present in first 6mths, respiratory distress
2
Hyperlucent lung
Initially soft tissue density from retained fluid
3
Normal/Reduced hemithorax volume
Overexpanded lobe
4
Bronchiectasis
Ventilated by collateral air-drift
Lt upper lobe > Rt middle lobe > Rt upper lobe

HRCT:
Reduced vascularity => Ipsilateral small vessel/hilum
Reduced lung attenuation
Air-trapping
Rx: Resection
